Abstract

Type 2 diabetes mellitus (T2DM) accompanied by obesity and multiple complications requires
individualized and integrated nutrition management. This case study aimed to describe the implementation of the
Nutrition Care Process (NCP) in a patient with obese T2DM, diabetic neuropathy, diabetic gastroparesis, febrile
suspected urinary tract infection, and mild hyponatremia. A descriptive case study was conducted prospectively
during hospitalization using the four stages of the NCP: nutrition assessment, nutrition diagnosis, nutrition
intervention, and nutrition monitoring and evaluation. Data were collected through dietary assessment,
anthropometric and biochemical measurements, clinical examination, medical records, and nutrition-focused
physical assessment, and were analyzed descriptively by comparing conditions before and during nutrition care.
The assessment identified excessive energy and macronutrient intake, frequent consumption of sugar-sweetened
beverages, limited adherence to previous dietary recommendations, obesity, and severe hyperglycemia.
Individualized nutrition intervention was provided through controlled energy and macronutrient intake, dietary
modification, nutrition education, and family involvement. During monitoring, dietary intake progressively
approached the prescribed requirements, while random blood glucose showed a marked decline during
hospitalization. Nausea and abdominal pain resolved, and the patient's general clinical condition improved,
although obesity and hyperglycemia persisted at the end of observation. The findings indicate that the NCP is a
feasible and clinically useful framework for delivering individualized nutrition care in patients with complex
T2DM and multiple comorbidities. Continued nutrition counseling, self-management support, family
involvement, and multidisciplinary monitoring are needed to sustain nutritional and metabolic improvements after
hospitalization.
